YEARLY FORMS AND DOCUMENTS

It is Wapsie Valley Community School's policy that, should you see a physician or other medical professional for a sport-related injury or injury that affects your sport, you will be required to have written clearance from said physician or medical professional to return to sport.  The written note given to you can be faxed to the school, but it is preferred that you utilize this attached form to take with you to your visit.  Wapsie Valley High School Fax: (319) 246-8529.
